On call nursing staff are available 24 hours a day, seven days a week.

Clinical Services

Chester River Home Care & Hospice offers care for patients with a wide range of special needs and conditions, including:

Cancer – For cancer patients who require skilled professional home care to enable them to focus on wellness, symptom relief and improved quality of life.
Cancer services may include chemotherapy, or care after radiation or surgery.

Cardiology – Provides care to individuals who have experienced a cardiac event or related surgical procedure with such services as patient education, diet instruction, and guidance in making lifestyle adjustments.

Diabetes – Provides the diabetic patient with disease education, equipment education, diet instruction and counseling in lifestyle changes.

Maternal/Newborn – After discharge from the hospial, home care nurses conduct a complete assessment of mother and baby. Other services include emotional support, early detection of any medical concerns and infant care education for new mothers.  This care also includes assistance for nursing mothers.

Pediatrics – Provides children and their families with a range of services, including teaching, clinical assessments, counseling and coordinating services with community agencies.

Rehabilitation – Provides physical therapy, speech therapy and occupational therapy to individuals who are recovering from a traumatic injury, stroke or surgery, as well as those who are experiencing complications of chronic disease or preparing for outpatient rehabilitation.

Hospice

The goal of the hospice program is to offer dignified, comprehensive and personalized care to patients and their families and friends.  This occurs during and after the final stages of life. Through a holistic approach, physical pain and other symptoms of illness are managed.  This allows patients and their families to resolve unfinished matters and enjoy the best possible quality of life in the time that remains.

The staff at Chester River Home Care & Hospice provides clinical services for hospice patients. Through the Kent Hospice Foundation, patients also receive support services such as transportation, volunteers, respite care, and grants to assist patients and families with supplemental medical supplies and other needs. Bereavement counseling and non-denominational spiritual counseling are also available.
