Chester River Home Care & Hospice Home care services are today’s version of the house call, but now services are provided by registered nurses, professional therapists and home health aides.  Guided by a plan of care designed by a physician, Chester River Home Care & Hospice provides care for patients in the comfort of their own homes. These visits may be needed before or after hospitalization or after inpatient rehabilitation. They may even take the place of a stay in the hospital or a skilled nursing facility.
